Abstract
Penelitian ini bertujuan untuk merancang dan mengembangkan sistem pelaporan Safety Management System (SMS) berbasis website yang dapat digunakan oleh civitas akademika di Politeknik Penerbangan Surabaya dalam melaporkan hazard secara digital. Latar belakang penelitian ini adalah belum
tersedianya media pelaporan hazard yang efisien,terstruktur, dan mudah diakses, yang berdampak pada kurang optimalnya proses identifikasi dan tindak lanjut terhadap potensi bahaya di lingkungan kampus.
Metode pengembangan sistem mengikuti lima tahapan utama, yaitu analisis kebutuhan, desain sistem, pengembangan, implementasi, dan evaluasi. Sistem dirancang menggunakan bahasa pemrograman PHP dengan framework laravel, serta basis data MySQL dan tampilan antarmuka berbasis Bootstrap 4. Hasil dari pengembangan ini adalah sebuah website pelaporan hazard yang memiliki fitur login pengguna, form pelaporan, unggah bukti, validasi admin, serta pemantauan status laporan secara real-time.
Hasil evaluasi menunjukkan bahwa sistem ini mendukung peningkatan efektivitas pelaporan hazard, mempermudah proses dokumentasi, serta meningkatkan kesadaran terhadap keselamatan di lingkungan Politeknik Penerbangan Surabaya. Sistem ini diharapkan dapat menjadi salah satu upaya
penerapan budaya keselamatan sesuai standar ICAO dan mendukung pengembangan Safety Management System di institusi pendidikan penerbangan.

This research aims to design and develop a website -based Safety Management System (SMS) reporting system that can be used by the academic community at the Surabaya Aviation Polytechnic for digital hazard reporting. The background of this research is the lack of an efficient, structured, and easily accessible hazard reporting platform, which impacts the less than optimal identification and follow-up of potential hazards on campus.
The system development method follows five main stages: needs analysis, system design, development, implementation, and evaluation. The system was designed using the PHP programming language with the laravel framework, a MySQL database, and a Bootstrap 4-based interface. The result of this development is a hazard reporting website that features user login, a reporting form, evidence upload, admin validation, and real-time report status monitoring.
The evaluation results indicate that this system supports increased hazard reporting effectiveness, simplifies the documentation process, and raises safety awareness within the Surabaya Aviation Polytechnic. This system is expected to contribute to the implementation of a safety culture in accordance with ICAO standards and support the development of a Safety Management System in aviation educational institutions.
